COMMUNITY-BASED ELECTRONIC HEALTH RECORD
STATOS improves the delivery of care in a
community
by systematically capturing and
coordinating results
to every provider.
STATOS provides each care provider with a
workflow
that can be adapted to their internal
operational needs.
- Workflow monitoring highlights include:
- Automatically alerting providers of every client’s
movement or change in status - Maximizing resources and reducing redundancy
through single source data entry - Monitoring every plan objective, for compliance
and outliers - Prompts for incomplete or required information
CARE PLAN BASED SERVICE ORDERING
- The level of care and type of service prescribed for clients are based on the results of their initial and ongoing assessments. The initial assessment provides the basis for a care plan that defines the objectives of their care, and required services.
- Services, such as counseling, transportation, medication review, etc. can be ordered directly from the care plan screens. Alerts and reminders prompt the user if a deviation or conflict occurs before the scheduled event.
-
STATOS synchronizes client schedules with other community systems, to resolve conflicts, reducing time and improving decision-making
- Historical documents, images and other media can be scanned, uploaded and attached to the client record.
CASE MANAGEMENT
Field casework is time and resource intensive. The personal
demands on care workers are greater in the field than they
are for service and care provided in the controlled environment
of a hospital.
STATOS allows Supervisor to balance the workload of field
caseworkers under their supervision. The Supervisor is then
alerted to redistribute workloads in order to avoid “case worker burnout”.
STATOS enables the supervisor to respond immediately to changes
in a patient’s level of care, caseworker absenteeism and scheduling
conflicts.
